Youthhealthtalk enables young people, their family and friends, and professionals such as doctors and teachers to understand young people's experiences of health, illness and life in general. The website feature real-life accounts of issues such as effect on work and education, social life and relationships, consulting health professionals and treatment.

New Patient Registration

registration1If possible, please call into the surgery to register with the practice.  You will be given a registration form together with a short health questionnaire.  We ask that adults provide photographic identification and evidence of  address such as a utility bill. 

We also offer the opportunity to attend for a new patient check with a practice nurse.  This includes the recording of height, weight, blood pressure and an analysis of urine (please bring a sample in a suitable container) and the opportunity to discuss your current medication and past medical history.  For patients that are too ill to attend the surgery, similar arrangements can be made to carry out a new patient check at home.

In order to maintain continuity of care and an even work load, all patients will be allocated to a GP on joining the practice.  However, you may express a preference to see a particular doctor but this will depend on the circumstances at the time of application.
