Reasonable adjustments

The NHS is required to ensure that all health services are equally accessible and easy to use for everyone, including people with disabilities.

This is delivered through measures known as reasonable adjustments, changes or support put in place to remove barriers and ensure you can access care in the same way as anyone else.

Requesting reasonable adjustments is your legal right.

These adjustments may relate to how your care is provided, or the practical support you need during appointments, for example when visiting your GP practice.

If you would like to request adjustments to support your care, please complete and submit a form on the link below or call us on 01297 20877.
